About us
Bring us your card — we will tell you what your plan covers
We are in network with Delta Dental Premier, Blue Cross Blue Shield Indemnity and Altus Dental. We accept all PPO (Preferred Provider Organization) plans and are happy to submit treatments for insurance reimbursements as a courtesy.
Dental benefits are set per plan, not per company. Two patients can hand us cards with the same logo and have different deductibles, different annual maximums and different coverage percentages. That is why we check your individual plan rather than going by the carrier's name alone.
Call the office with your carrier, member ID and group number and we will check your benefits before your visit, then explain what they mean in plain language.

About us
What we need from you
In network means the practice has a contract with your plan that sets the fee for each procedure. Out of network means no such contract exists — your plan may still pay toward the visit, usually at a lower percentage, and the remainder is yours. Either way, we tell you the estimated out-of-pocket amount before treatment starts.
Your carrier
The carrier name shown on your dental card.
Member ID
Your member or subscriber ID, and the group number if your plan is through an employer.
Date of birth
The subscriber's date of birth, if that is not you.

Proceed Finance
